Slide Rock State Park Team Building Field Trip July 18, 2023

Dear Parents,

Welcome back to a new school year. If you are new to Ash Fork middle school or are a veteran parent, I would like to take the time to talk to you about team building. Team building is an integral part of Ash Fork middle school and occurs once every quarter. The goal of team building is to allow all students to have the opportunity to try new things that may be outside their comfort level. As a school, we encourage these fun and challenging situations because it is only through difficulty that we grow and learn. For our first team-building activity, we will take the students on an easy hike to Slide Rock State Park. During these activities, students will have the opportunity to bond as they hike and swim.

Slide Rock State Park is located in Oak Creek Canyon on a 43-acre apple farm. The team building hike is 0.3 miles one way and 0.6 miles round trip and will start near the apple packing barn. During the walk, your child can swim and slide in the pools found within Slide Rock State Park. The school will be providing lifejackets for students and a lunch. If you would like to find more information about the trail you can visit Slide Rock State Park | Arizona (azstateparks.com).  Please read the following details so your child can enjoy the hike. 

When: July 18, 2023

 Itinerary: Be at the school by 7:15 A.M. We will leave at 8:00 A.M. We will return to school by 4:00 P.M., therefore your child will be able to ride their regular school bus home. 

 Clothes: Your child will need two pairs of shoes, one for hiking and one for getting wet. No Crocs. The shoes must be closed-toed, and your child must wear socks during the hiking. Sliding on the rocks within in the park can rip clothing. Your child should wear shorts that are okay for sliding on the rocks. Your child should wear clothes that will dry quickly when wet or bring a dry pair of clothes they can change into. 

 Other essential items: Ensure your child brings a trash bag for their wet items. Your child should also bring a backpack, water bottle, hat, and sunscreen. 

 Food: The school will be providing a sack lunch for your child. If they would like to bring a snack in addition to their lunch, they can. However, we ask that they bring no more than two additional snack items.

  Please sign and return the permission slip on the back of this form. There are two places to sign. The first is permission to attend the trip, and the second is permission for the cliff jumping. Your child does not have to participate in the cliff jumping to go on the hike or swim in the creek.

 As the parent or legal guardian of the above-named student, I give permission for my child to participate in the Slide Rock State Park Trip (the Trip), which will take place on July 18, 2023.

Participation in this Trip involves being in bodies of water, including rivers and pools that may involve risk of serious injury, including drowning, permanent disability and death and damage to personal property, which might result not only from my child’s own actions, inactions or negligence, but from the actions, inactions or negligence of others or the conditions of the premises or of any equipment used. This Trip also includes a hiking component in the outdoors, so my child will be exposed to various risks typically associated with hiking in the outdoors. Such risks include uneven terrain, adverse weather conditions, exposure to insects, reptiles, and plants, and swimming in a body of water. These activities also involve risks of injury or death or damage to personal property. Knowing the risks, I give my child permission to participate in these activities.

 I give permission for Ash Fork School District employees or volunteers to seek medical treatment for my child as necessary in their judgment. I am aware that Ash Fork School District does not provide accident or health insurance coverage for my child and have independently determined whether I should obtain such insurance at my own cost. I understand that the student’s accident or health insurance or the parent of the student will pay for any health care and expenses incurred. Payment of such expenses is not the school district’s responsibility.

 I understand that my child’s participation is conditional on maintaining proper and respectful behavior towards the school district staff, volunteers, and other students. The same code of conduct required of my child while he or she is in school applies to this Trip. If my child misbehaves, then he or she may lose the privilege to participate in the activities of this Trip. I have read and understand the foregoing information and acknowledgments. I understand that this agreement is legally binding between the parties outlined above and affects any legal right to claims for damages in the event of injury, illness, damages or death. My agreement to my child’s participation is voluntary and without duress.
